Practical support and human community, together

At the practical level, Vibrant Health Advocates - Sable runs structured self-management programmes and peer support sessions across Elgin, all of them free to attend and designed for adults living with any form of persistent or chronic pain.

Our core Self-Management Circles follow a six-week curriculum grounded in the Stanford Chronic Disease Self-Management Programme model, adapted for our community and delivered by trained peer facilitators who have their own experience of long-term pain. We also run weekly gentle movement sessions, specialist flare-planning workshops, and a monthly ongoing peer group for people who have completed our programmes.

Referrals come through GPs, physiotherapists, occupational therapists, and self-referral โ€” anyone living with chronic pain in the Moray area is welcome to get in touch. Beyond our direct programme delivery, we work to reduce the isolation that so often compounds pain. We provide a supported transport scheme for participants who cannot easily reach our Elgin venues, we maintain a resource library of books, audio programmes, and printed guides that participants can borrow, and we offer one-to-one conversations with a trained peer for people who are not yet ready for group settings.

We also work at a systems level โ€” sharing anonymised learning from our participants with NHS Grampian and Moray Council colleagues, contributing to local health planning conversations, and advocating publicly for chronic pain to be recognised as the significant, widespread public health issue it is.

Four structured ways to build your own toolkit

Every programme is free, every programme is peer-led, and none require a GP referral โ€” though we welcome referrals gladly.

Self-Management Circles

Our flagship six-week peer programme, equipping participants with a personal toolkit for managing chronic pain day to day.

Each circle brings together eight to twelve people for a structured six-week course, facilitated by two trained peers who live with chronic pain themselves. Sessions cover pacing and energy management, understanding the pain cycle, sleep and rest strategies, communicating effectively with healthcare professionals, and goal-setting that accounts for fluctuating capacity. Participants leave with a personalised self-management plan and an ongoing connection to the Sable community.

Gentle Movement & Wellbeing

Weekly drop-in sessions blending adapted movement, relaxation techniques, and informal peer support in an accessible Elgin venue.

Led by a qualified instructor with specialist training in chronic pain, these sessions are explicitly designed for people on difficult-pain days as much as easier ones. We use chair-based yoga, progressive muscle relaxation, and breath-focused practices adapted from pain management research. The post-session social time is as valued as the movement itself โ€” many participants describe it as the one hour each week that is entirely their own.

Flare Planning Workshops

Practical half-day workshops helping participants create robust, personal plans for managing pain flares before they happen.

Flares โ€” those periods when pain intensity spikes and normal functioning feels impossible โ€” are one of the most destabilising aspects of living with chronic pain. Our Flare Planning Workshops help participants identify their personal early-warning signs, build a tiered response plan, and communicate their needs clearly to family, carers, and employers. We provide a printed Flare Plan booklet that participants complete during the session and take home.

Connect & Thrive: Ongoing Peer Group

A monthly open gathering for anyone who has completed a Sable programme, sustaining connection and sharing new strategies over time.

Recovery and self-management are not linear, and community support should not end with a six-week course. Connect & Thrive meets on the third Tuesday of each month at a central Elgin venue and is open to all past participants. Sessions are informal but purposeful โ€” a facilitated check-in, a short skills refresher, and open peer conversation. Many of our most experienced participants describe Connect & Thrive as the programme that holds everything else together.

Supported Transport Scheme

We know that getting to sessions in Elgin can be genuinely difficult for people managing chronic pain, particularly for those travelling from more rural parts of Moray. Our supported transport scheme is there to make sure that distance and mobility are not barriers to attendance. Contact us to find out if you qualify and how it works in practice.

Resource Library

Our resource library holds a curated collection of books, audio programmes, and printed guides on chronic pain self-management. Participants can borrow from the library at any time, and staff can recommend materials tailored to particular conditions or goals. We update the library regularly based on participant feedback and emerging research.

One-to-One Peer Conversations

Not everyone is ready for a group setting โ€” and that is completely understandable. For people who want support but aren't yet comfortable joining a circle, we offer one-to-one conversations with a trained peer facilitator. These are informal, confidential, and designed simply to help you explore what might work for you. There's no expectation that a one-to-one will lead to group attendance, though many people do find it a helpful first step.

NHS & Partner Referrals

We work closely with NHS Grampian colleagues, Moray Council social care teams, and local third-sector partners to ensure people find their way to us. GPs, physiotherapists, occupational therapists, community nurses and many other health professionals refer regularly to our programmes. Self-referral is equally welcome โ€” you do not need a professional referral to attend any of our sessions.
